Step-by-Step Guide to Applying for the CDCP

1. Check Eligibility

Before filling out your Canadian Dental Care Plan application, ensure that you meet the eligibility criteria for the CDCP. Refer to the Eligibility section to verify your qualification based on adjusted family net income, your access to any private dental coverage, disability status. Specific age-based restrictions may still apply.

2. Gather Required Documentation

To complete your Canadian Dental Care Plan application, you will need the following information for yourself and your spouse or common-law partner (if applicable):
  • Social Insurance Number (SIN)
  • Date of Birth
  • Full Name
  • Home and Mailing Address
  • List of any dental coverage through government social programs (if applicable)
  • Proof of tax filing for the most recent year (Notice of Assessment)

    3. Application Process

    Applications for the CDCP open for all eligible Canadian resident at the same time in 2026. 
    • New CDCP Applications: June 2, 2026.
    • Renew CDCP: April 15, 2026 – June 1, 2026.
    • When your coverage will start: Canadian Dental Care Plan (CDCP) operates on an annual benefit year running from July 1 to June 30.

    • Alternatively, you can call Service Canada directly at 1-833-537-4342 (or TTY at 1-833-677-6262) between 8:30 AM and 4:30 PM local time.

    After You Apply

    After submitting your application:

    • Enrolment Confirmation: You will receive a confirmation of your enrolment from Sun Life, the administrator of the CDCP.
    • Welcome Package: Within three months, Sun Life will send you a welcome package that includes your member card and coverage start date
    • Choose a Registered Provider: Search for and select a local dental clinic that actively accepts CDCP patients.
    • Estimation & Co-Payments: Before booking an appointment, confirm that your dental clinic actively participates in the CDCP. When booking, ask if there will be any balance billing or co-payments, as the CDCP pays according to established government fee guides and the clinic prices can be different.
